For spas that offer medical skin treatments, they need the proper certification, but those services are not offered at day spas. The cosmetic services offered at a day spa do not require licenses, but most aestheticians and massages therapist at day spas still obtain a license.
When you go to a day spa, you will typically receive a robe, slippers, tea, water, fruits, nuts, and access to locker rooms, the resting area, and more. Other types of spas usually do not offer amenities like these.
